HopkinsBasic books on family history/genealogy that you can borrow at your local public library may also have charts that you can copy. If you look at cyndislist.com - under the topic "Supplies, Charts, Forms, Etc." and you'll find a long long list of places to get such forms.
Jo Anne SadlerFamilysearch.org has blank forms AND free downloads of their Personal Ancestry File program that is very easy to use and compatible with all other family tree programs.
